Yeh Hai Mohabbatein starts with Raman(Karan Patel) driving towards meeting Rayna. Raman reaches the spot and is closely followed by Ishita(Divyanka Tripathi). Raman meets Rayna who asks him if he is ready to kill her husband. Raman says that if has to kill someone then it would be Rayna herself. He points his gun at her and she laughs at him saying she knows he cannot kill anyone. Ishita reaches the spot and finds Raman with his gun. She runs to stop him. Raman asks her to move away as he would finish Rayna today who tried to take away his family from him. Ishita runs to him as he almost pulls the trigger and the bullet hits Ishita.

Raman runs to her and Rayna stands stupefied. We see an accomplished Simmi hiding away behind some trees. She has a gun and it is revealed that it was her who shot Ishita and not Raman. Simmi smiles to herself. Raman tries to wake Ishita who has been shot in head. He cries to her asking her get up as she is the only one who can take care of his family and especially Pihu. Rayna who stands their shell chocked does not move and Simmi signals to her to run away. Later Simmi takes the opportunity to lock up Raman's car after placing the gun in her hand inside the car. Adi and Romi are searching around for Ishita when they try to call her again and again. Adi gets more and more worried about his mother. They sees Ishita's car and stop by but do not find her anywhere near. They drive further and find Raman with Ishita in his arms on the side of the road as he tries his best to stop the cars and find help. Adi and Romi jump out of their car and run to the couple as Raman keeps repeating to them that he shot Ishita. They pick her up and take her to the hospital. At bhalla house everyone worries over Raman as Santoshi says that she feels something bad has happened. Aliya keeps trying Romi and Adi's numbers. Madhu gets up as she gets worried about Ishita now and says that no one here seems concerned about her at all. Simmi walks in and says that she was out looking for Raman but did not find him.

Aliya gets Adi's call who informs her of the situation and everyone rushes to the hospital. Simmi meets Param who wonders who actually shot Ishita as Simmi does not tell him a thing yet. Raman with Adi and Romi get Ishita to the hospital. Raman keeps repeating about how he shot her and it was not intentional. Ishita is taken in and the family rushes in too. Raman tells everyone that he shot Ishita. Madhu lashes out at him and asks him what Ishita did to deserve this. The family asks everyone to calm down. The police arrive and Raman tells them he is the culprit but Santoshi refuses to believe this and says that Raman can never do this. Ruhi arrives the place too. Simmi takes Param aside and tells him that it was she who shot Ishita and now she has finally avenged her child. She also says that she has paid Rayna and asked her to leave town. Simmi and Param are confident that Ishita will not survive and Raman will either go to jail or the asylum.

Romi and Mihika take the police aside and tell them that Raman is disturbed as Ishita is in the condition and also Raman is undergoing medication for mental disorder. They ask the police to overlook Raman's confession and proceed with their own investigation. Later the police permit Raman to go home and Param is quite disturbed by this. Raman reaches home with Adi and Romi but he asks them to go back to the hospital and look after Ishita. He says he would be fine alone. As the duo leave Raman too stands up and looks at the pictures of Ishita with the kids. He remembers the time spent along with her and feels sorry. He thinks that his disorder has reached Ishita to the verge of death and he decides to do something for which he seeks forgiveness of Ishita.
